Work in English
If you have the opportunity to work in a place that English is a requirement, you have to get that job! It's necessary practice everyday until this gets normal to you. You'll be forced to speak in English the whole day and it will become familiar for you, you'll become confident with yourself. You'll prove yourself everyday, having conversations in English and this will help you to reinforce your language. You're going to have relationships with other persons that speak this language too, so they will be persons with the one you can practice. Even if you're at lunch time or break time, even if you're not at work, speak to them in English, if you go out with them, forget Spanish, talk just in English.
Difference accents by region
As you know, the English has not the same accent everywhere. You'll need to recognize and understand the accent of the person you're talking to. Is not the same, talk with someone of the United States, than talking to someone of England. Sometimes is harder to understand the British English because of their pronunciation and in English schools most of the time they teach you American English and this makes it easier to be understanded. But be careful! Not every area speaks with the same pronunciation, it will be better for you if you practice with persons that pronounced in a different way for prove yourself how much you can understand.
Learn abbreviations and sayings
Every country has its own sayings, so you'll need to make investigations about the most common phrases and sayings of the country. You're going to need to know too, the abbreviations they used to use, in case you have a conversation with a native person you can understand what they say even if they abbreviate words. Start talking the same way they do, start using the most common abbreviations and sayings and you'll get used to them and this will make your language be better and extended.
Think in English
Stop translating and speaking Spanish! When you're thinking about something or if you're alone, think in English not in Spanish. Every time you have the opportunity of speak in English, do it! Make plans in English, write notes in English too, if you're going to the supermarket do your list in English and you'll start watching your advances. Make this language part of your daily routine and it will become easier for you and this will make you more confident.
Speak, speak, speak
Try to speak in English every time you can, if you know other people that speaks this language, talk with them in English, if you practice this way you can improve your language and get better skills while you listen the others pronouncing and you will develop better audition so it will be easier for you to understand complete conversations about different topics, formal and informal conversations, you'll understand and use phrases. You can give directions and orders and you'll be able to understand it too. You'll can describe things, places using different verb times using advanced vocabulary.

Traveling
If you have the oportunity to visit another country, you should go to a country that speaks English so that you can practice what you've been learning this time. You'll read and speak everything in English and that its going to help you a lot. Having conversations with people that English is their language since birth will be a helpful tool for you to improve your language skills, you're going to listen pronunciations that you may didn't know, you will know new words and your are going to practice with the vocabulary that you have learned and with the audition you have developed to see if you can understand everything they might say to you.
Look for practices
Start looking for online practices or book practices, complete them and then check your grades so you will know how good is your grammar and what you have to practice more and reinforce. Ask someone for help, somebody who could make a daily practice for you maybe. A practice that isn't make by yourself is always a helpful way, because if you do a practice for you, you may will put the things that easier and that you know more and that's not the idea. Other wise, if someone make the practice for you, they will do the practice for whatever they chose, and you will have to do your best.
Reading books
Try reading books, in English of course, this way you're going to see how words are written. When you do this, try understanding complete sentences or paragraphs, do it with a dictionary, so if you don't know the meaning of a word you look for it inmmediatly for understand what the text is saying and for learn a new word's meaning. You could also have a paper, and go writting down there the new words you learned and its meaning in Spanish.
Some good ideas are to try reading the book in Spanish even if it's written in English, so you will be proving your level of vocabulary and how much you can understand, and when you can't understand something you look for it in the dictionary, but don't do it without it just in case you need to look for a word.

English music lyrics
If you listen music in English, this will help in different ways. Your vocabulary will increase, your pronunciation will be better because you'll be listening how words are pronounced and your audition will be clearer. You'll get used to listen fast things and after a few times, you will understand what they are saying, so when you're having a conversation it won't be hard for you understand if the other person speaks quickly.
You can also translate the songs, you will learn new words and its meaning and then when you listen the song again you will know what it's saying and you will understand it in an easier way. After doing this sometimes and having more vocabulary, you're going to understand without the necessity of translating.
Watch English movies and T.V programs
A good advice I can give you is to start watching movies and T.V programs in English, this will help you to reinforce your audition and even pronunciation. You can also use the subtitles so you will know the meaning of a word that you didn't know. You can start, if you want, with some child programs because the vocabulary it uses is not as advanced as a program to grown ups.
You will start noticing your advances, when you can understand more and more the things you watch, you will understand complete sentences and even complete conversations, not just words and that's one way you'll know you're progressing.
